Granite Creek Health and Rehabilitation Center is a modern, state-of-the-art skilled nursing facility, located in central Arizona in the historic town of Prescott. We are just east of Prescott High School on Scott Drive next to the senior living campus of Las Fuentes Resort Village.
We feature 128 beds and beautiful spacious rooms in either private or semi-private settings. Guests will enjoy well-appointed accommodations, a diverse activities program, nutritious satisfying menus, housekeeping and laundry services, and beautifully landscaped grounds.
